Survey and Economic Research on Dentistry reports are available for purchase as printed, bound reports as well as downloadable, electronic files for your convenience through the
ADA Catalog. The following describes how to place an order. Please note that all orders must be prepaid.
Pricing
Prices and current years of publications and reports can be found in the Publication Price List PDF file/42k  or by visiting ADA Catalog. There are five levels of pricing: Member, Non-member Dentist, Commercial, Constituent Dental Society, and International. Prices are subject to change without notice. Please contact the ADA Member Service Center with any questions concerning eligibility for certain price levels, current years of publications, or ordering.
How to Place an Order
Online: Go to ADA Catalog and find the category Survey and Economic Research on Dentistry under the Browse ADA Catalog tab to the left of your screen; reports may be found in any of the topic areas listed there. You may also search for publications by typing keywords in the Search box.
To order online, you will need to log into the Catalog. Please note that all dentists, members and nonmembers, as well as dental students must use their ADA Number as their Customer Number and their 5-digit primary address zip code as a password. All other buyers may create a new user profile if one does not already exist. If you have questions regarding how to log in or how to create a user profile, please call the Member Service Center at 800-947-4746 for assistance.
By phone: Call our Member Service Advisors at 800-947-4746 weekdays, 8am–5pm CST. Please have your order information, ADA membership number (if applicable) and Visa or MasterCard on hand.
By Fax: Print and fill out the Survey Center order form PDF file/103k . Enter the item number(s) found in the Publication Price List. Fax your completed order form (paid by Visa or MasterCard only) to the Member Service Center at 312-440-3542. Please include your ADA membership number (if applicable), fax and phone numbers so that we may contact you if questions arise concerning your order.
By mail: Print and fill out the Survey Center order form PDF file/103k . Enter the item number(s) found in the Publication Price List. Mail the completed order form, along with a check or money order, to the address below. Payment may be made to the "American Dental Association." You may also use your Visa or MasterCard. Be sure to include payment with order.

Shipping Information
Orders shipped within the continental U.S. are normally sent via Fed Ex. States and U.S. Territories outside the continental U.S. will incur higher shipping charges. Be sure to include a street address and phone number for easier delivery.

International Orders
Please go to ADA Catalog or call 312-440-7727 to place an order from outside the U.S. and its territories. We strongly encourage international customers to order downloadable electronic products (where available) in order to avoid duty, tariff, and tax charges. All international orders (including Canada and the Caribbean) must be prepaid through Visa or MasterCard.

Guarantee
Your complete satisfaction, both in the quality of merchandise and in the way we handle your order, is very important to us. If you are not completely satisfied with your ADA© product, simply return it within 30 days providing your reason for dissatisfaction and we will gladly refund your money or provide you with an alternative solution.

Returns and Exchanges
If you choose to return an item, please call our Member Service Advisors to expedite processing. Include customer name, order number and reason for return and let us know if you want a replacement, refund, or an exchange. Original shipping charges will not be refunded.
If an item arrives to you damaged or is not what you ordered, please contact our Member Service Advisors immediately at 800-947-4746 for a replacement.
